Position: Transition Coordinator / SpecialistDivision: Pupil Personnel Services (PPS)Supervisor: Director of Pupil Personnel ServicesEmployment Term: 10 monthsFLSA Status: Exempt SUMMARYCoordinate Transition Services for Lexington School for the Deaf students. Work with Lexington Faculty, Staff, and community partnerships to develop an appropriate transition plan for each student to ensure a successful transition to post-secondary education and opportunities. DUTIES/RESPONSIBILITIESAbility to work collaboratively with colleagues, families, and the communityCoordinate ACCES-VR student referrals and services for students and families starting at age 18Refer and assist families with OPWDD eligibility and applicationsAssist families with information and referrals for work incentive benefits counselingWork with students and their families on travel issuesCoordinate work experience opportunities and programs to include both on-campus and off-campus placementsWork with faculty and staff to provide pre-employment skills and transition preparation to High School students participating in work experiences during school hoursCollaborate with teachers, counseling staff, and related services personnel in planning and implementing effective transition planning and IEP goalsSupport and consult with students’ families on transition planning issuesSet up transition-related workshops for students and families to access resources to be used for post-secondary students’ successTracking students graduates for data and to determine the effectiveness of our school’s educational programming and servicesREQUIRED SKILLS/ABILITIESExcellent verbal and written communication skillsExcellent interpersonal and communication skills including proficiency in English and American Sign Language (ASL)Excellent interpersonal skillsExcellent organizational skills and attention to detailExcellent time management skills with a proven ability to meet deadlinesStrong analytical and problem-solving skillsAbility to prioritize tasks and to delegate them when appropriateProficiency in American Sign Language required.Proficiency in Spanish preferred, willingness to learnProficiency in Microsoft Office Suite and related software EDUCATION AND EXPERIENCEBachelor’s degree in human services, psychology, or related field requiredThree years of experience in career and vocational training with youths SECURITY CLEARANCERequired clearance includes medical clearance, fingerprinted by the NYC Board of Ed, and SCR and SEL clearancePHYSICAL REQUIREMENTProlonged periods of sitting at a desk and working on a computer.Prolonged periods of walking around in and outside of the building.Must be able to lift 15 pounds at times.
