Position: Career and Transition Services CoordinatorDivision: Curriculum and InstructionSupervisor: Upper School PrincipalEmployment Term: 10-month position + ESYCFR Code: 255 SUMMARYResponsible for assisting the students in areas of academic, achievement, personal or social development, and interested and growing to become a well-adjusted and productive adult. Essential Functions: Collaborate with families and school staff to assist Lexington students with their transition plan that aligns with the student’s career interest and skills. Informs students, families, and school staff on graduation credential requirements and post-secondary options. Participates in Family Conferences throughout the year. Provides career counseling services to the students. Support students with the collegiate application and financial aid process. Prepares Pre-Employment Transition Services for Juniors and Seniors (i.e., Career Exploration, Workplace Readiness Skills, Post-Secondary Options, Self-Advocacy Skills, and Work Based Learning). Monitors students’ portfolio, documenting a history of achievement that students build from K to 12. Conduct and monitor vocational assessments to assist students in identifying their career and academic goals with the support of Transition Specialist. Coordinate in the planning and delivery of programs including summer programs (i.e., SYEP/NTID Regional STEM Center), seminars/workshops, post-secondary training program visits, and college presentations. Coordinate internship placements during school and SYEP and conduct work readiness evaluations. Participate in transition exit review meetings with school staff, families, and students to finalize transition plans and responsibilities. Monitor progress of students’ Career Education IEP goals and identify each student’s exit credential/diploma status that are aligned to post-secondary transition goals. Focuses on IEP transition goals including information on credits and diploma track. Assist with the Testing Coordinator to provide students college entrance exams opportunities (i.e., ACT). Maintains accurate documentation and track CDOS requirements in students’ transition portfolio and permanent records. Work with schedule masters on class scheduling to meet students’ course of study credit requirements including internship scheduling. Assist the Upper School Principal in developing career pathways including dual credit programs, and overseeing and fulfilling NYS graduation requirements. Consult a Transition Specialist with ACCES-VR, OPWDD, and/or other appropriate agencies to develop a strong exit transition plan. Effectively implements a data-driven, post high school graduation tracking system. Performs duties as assigned by the Upper School Principal. Knowledge, Skills & Abilities: Ability to communicate effectively with teachers, school staff, parents, administrators and deaf students using ASL and English. Ability to work effectively and collegially with others and to lead others, when appropriate. Willingness and ability to demonstrate commitment to the school vision and mission. Possess flexibility and an appreciation and respect for up-to-date and on-going research in the field of Deaf Education and data-based decision-making. Recognition of self as a learner, ability to identify areas for growth and commit to working in these areas. Qualifications: New York State Teaching Certification. Master’s degree in Deaf Education or related field. Physical Requirement: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift 15 pounds at times. Security Clearance: Required clearances include medical clearance, fingerprinted by the NYC Department of Education and SCR and SEL clearance.
