QSAC is looking for a focused and dedicated Transition Coordinator to support our day school in the Bronx. This position works with the students, clinicians and families to assist in future planning for students at the school. Candidate must be a certified Special Education Teacher, LMSW, Psychologist or Certified School Administrator.ResponsibilitiesDevelop Transition plans for school age students ages 14-21Conduct vocation assessmentsCoordinate vocational settingsEnsure post-secondary school/ linkage referrals for graduating studentsCoordinate school and local resources to provide vocational opportunities Work Closely with ACCES-VR to provide supports to graduating studentsParticipate in parent training clinics as neededServe as substitute teacher as neededProvide and/or ensure regular communication with parentsMaintain regular communication with classroom and administrative staff regarding student progress, staff progress, and overall class managementMonitor and graph performance/productivityParticipate in the evaluation of staff as neededAttend/participate/develop classroom and or school-wide trainingsAssist/guide/mentor in the development of annual narrative reports/quarterly reports.
