The LaSalle School is seeking a Transition Coordinator/Work-Based Learning Coordinator (Full-Time Exempt, 40hrs per week, 12-months). This position will report to the Associate Director of Educational Services and Director of Educational Services, as needed.The role of the Transition Coordinator/Work Based Learning Coordinator is to deliver program services which help to prepare students to acquire skills which will enable them to successfully transition to work or appropriate post-secondary studies. This includes coordinating and supporting student work experiences both on and off campus, developing viable work sites for our students, and staying current with best practices regarding school to work transition strategies. When appropriate participate in planning and assisting for appropriate post-secondary educational opportunities. This person will need to be familiar with the relationship between CDOS standards and normative experience guidelines as identified by OCFS ensuring appropriate documentation of those activities is completed. In addition, this person will participate in planning and delivering programing for CDOS credential students.Job Responsibilities (only illustrative and not all inclusive):Teach employability skills classes and provide other appropriate job preparation instruction to students.Provide a resource for working papers and assist student to secure and maintain employment both on and off campus jobs. Coordinate the Summer Job Corps program with the City of Albany.Monitor daily activities of the working students and provide the support necessary to help each student successfully attain his goals.Work closely with the Educational Services Team, Day Services Team and Residential Services Teams, to assure school to work efforts are carefully integrated into the overall treatment plans and IEPs for each student.Coordinate the career assessments, vocational assessments, and all SED (CSE) required transitional services for students.Maintain all necessary documentation and adhere to all related time lines. (Assessments, CDOS, normative experiences).Work closely with ADI to assure curriculum development in all subject areas includes aspects of school to work skills and CDOS requirements to foster acquisition of subject related skills necessary to meet employability goals.
