QUALIFICATIONS: Current/Valid New York State Certification as a School Social Worker or School CounselorREPORTS TO: The School-Home Liaison reports to the building principal of the assigned school and indirectly reports to the Director of Student Support Programs ESSENTIAL JOB FUNCTIONS:Provides services to students and families to address any issue that interferes with the educational process using goal-oriented casework, group work and community organizational skillsProvides short-term interventions for students and familiesMeets individually or in small groups with students to provide counseling, referrals, educational planning and goal settingConducts home visits, facilitates parent education and serves as a resource for familiesServes as a member of building or district intervention teams. Provides information in school support Maintains records and information obtained through individual contacts and makes necessary Maintains current information regarding community resources and makes appropriate referrals in situations where a referral is warranted. Empowers students and their families to gain access to and effectively use formal and informal community resources.Works collaboratively to mobilize the resources of the local education agencies and the community to Collaborates with teachers, psychologists and other school personnel to identify children in need of services. Utilizes a process of needs assessment planned consultations with staff, children and families and community resources.Following an assessment, develops and implements a plan of intervention that will enhance children’s AIS monitoring for students with attendance and/or behavioral issuesPerforms other related duties that may be requested by administration.
