The Valley Stream Central High School District, a beautifully diverse school community, is currently seeking candidates for the position of Special Education Transition Coordinator.Essential responsibilities include the following:Working with classified special education students to achieve the transition goals set in their Individual Education Plans (IEP’s)Liaison between the student and family, the district and appropriate state agencies such as the Office for Persons with Developmental Disabilities (OPWDD) and Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR).Coordinating with in-district and out-of-district students who are working toward completing the requirements to earn a Career Development and Occupational Studies (CDOS) graduation credential.NYS certification as a school counselor, school social worker or special education teacher strongly preferred.
