SMITHTOWN CENTRAL SCHOOL DISTRICT OFFICE OF CERTIFIED PERSONNEL Date: April 14, 2026 Posting #: CP3009 FAST PROGRAM/TRANSITION COORDINATOR ANTICIPATED PROBATIONARY POSITION FOR THE 2026-2027 SCHOOL YEAR The District anticipates hiring a probationary teacher for the 2026-2027 school year to support the continued implementation of the Functional Academic Secondary Transition (FAST) Program and lead transition activities for students and families K-12. This position requires two (2) additional work days outside of the regular calendar of teacher work days. A valid NYS teaching certification is required. DESIRABLE QUALIFICATIONS: Knowledge of the Functional Academic Secondary Transition : FAST program (Life Skills). Experience teaching special education or experience in other PPS roles. Prior experience working with students who are alternatively assessed. Experience and knowledge of OPWDD, ACCES VR and post secondary transition services. NYS certification as a Special Education Teacher, Speech Teacher, Social Worker, School Psychologist preferred. FAST COORDINATOR RESPONSIBILITIES INCLUDE, BUT ARE NOT BE LIMITED TO: Assist the Assistant Administrator of HS Special Education and building administrators with FAST scheduling. Assist with NYS Indicators. Review all IEPs for students within the program to ensure alignment of mandates and program. Coordinate work study and transition programs with outside agencies, out of district programs and BOCES. Oversee the PAES Lab within High School West. Coordinate placement and coverage for all Special Education Aides at High School West. Complete Vocational Assessments for students in the FAST program. Assist staff and students with completing Transition Assessment Surveys. Coordination of OPWDD, ACCESS VR, LEAP and K-12 all buildings. Coordination of Behavioral Consultants for those within the FAST Program at HSW. Chairperson of CSE meetings for FAST students and attend CSE meetings for FAMP students as necessary. Develop and implement programs to assist and communicate with parents of students in the program or that need assistance with transition activities. Prepare for the 2027 ESYP summer program during the 2026-2027 school year. Other tasks as assigned by administration.
