Position: Transition CoordinatorQualifications:Required: Valid NYS Teaching CertificatePreferred: Experience working with Access VRCompensation: Stipend $3,129 Duties:Responsible for contact and communication with the Office of Early Learning at NYSEDWork with OPWDD Services;Meet directly with students with disabilities to file applications to Access VRUnderstand the transition service requirements as related to students’ post-secondary goals and coordinated transition activities;Assist special education teachers with implementing and monitoring a school-wide plan to provide appropriate and effective transition services;Act as the transition contact person for administrators, teachers, parents, and community partners;Collaborate with secondary staff in the preparation of the IEP and transition services;Work collaboratively with agency representatives to provide information sessions for students and parents, conduct eligibility and application activities, and work to transition students from public school services and supports to adult services and supports;Conduct regular evaluations of transition services effectiveness and compliance;Work with Life skills teacher to organize and schedule student job assignments and transportation to and from work-based learning experiences and meet with employers/employees at non-paid, work-based learning sites prior to placing students to establish a meaningful learning format, assessment process, collaborative support system;Organize and conduct a transition night for families whose students are transitioning to different schools. i.e. 6th-7th grade, 8th-9th gradeParticipate in the County-Wide Transition CouncilPerform other duties, as assigned, by the Superintendent of Schools.Requirements:Valid NYS Teaching Certificate;Experience working with Access VR; andInternal candidates should send a letter of interest to akerr@saugerties.k12.ny.us
