The North Rockland (Haverstraw-Stony Point) Central School District is seeking candidates for a full-time probationary Transition Coordinator position for the 2025-2026 school year. This position includes a highly competitive salary and comprehensive benefits package. Valid NYS certification in Special Education, Students with Disabilities (All Grades or Grades 7-12), School Counselor, School Social Worker, or School Psychologist is required. At least three years of professional experience in a secondary level school setting is preferred.Professional Duties:Consult with campus special education staff regarding students’ educational and transitional needs; Assist with the completion of ACCES-VR application and student transition surveys; Support campus special education staff in developing and maintaining effective transition interventions, assessments, goals and programing to meet the needs of their students; Serve as a resource for training special education staff, parents and community on the transition process, guardianship and other transition related activities; Provide staff development to building and district staff related to transition; Maintain accurate and complete written records in accordance with Transition and Work-based Learning requirements; Facilitate the auditing process for State Performance Indicators 13 and 14; Consult with building personnel and provide support for work-based learning (WBL), employers and pre-vocational programs for students with disabilities;Conduct Person Centered Transition Assessments/Functional Vocational Evaluations and develop written reports specific to student outcomes;Provide activities to classified students on a regular basis re: transition topics (weekly or monthly “seminars”) including:Life Skills Post-Secondary Planning Self-Determination Work Based Skills Resumes/ Applications/ Interviewing Skills
