Program: Case Manager Specialist Requirements: Must hold one of the following: Licensed Master’s Social Worker (LMSW), Licensed Clinical Social Worker, (LCSW), Licensed Mental Health Counselor (LMHC), Psychiatric Mental Health Nurse Practitioner (PMHNP) or Bachelor’s/Master’s in related field with experience with case management and care coordination Dates / Hours: 7/1/2024 to 10/31/25 Monday through Friday 8:20 am to 3:40 pm (excluding school holidays and not to exceed 225 days per year) Hours flexible to include after-school/ evenings and/or Saturdays as needed Provide case management, coordination of services for families with more than once child with SEL needs and or families who have or need multiple agencies or providers involved in their child’s care Provide referrals and support to families in need of SEL servicesAttend Trainings/workshops on the Westchester County CCSI Family Network process Coordinate and/or facilitate Family Network Meetings Serve as a liaison between families, school clinical staff and outside agencies Support families with scheduling and following up on SEL and/or medical appointments and recommendations Support families and or provide education on proper medication management in conjunction with the school nurses Provide Family training sessions and workshops Support families with intake processes to receive support and services, hospitalizations, therapeutic programs and/or discharge planning when neededCollect data and write session notes on all cases and clinical work providedConduct risk, threat assessments as neededAssist with providing professional development and workshops to staff and families as needed Assist the Director of Special Services with any/all clinical tasks and duties as needed to support the District’s Peekskill Promise and Whole Child SEL Initiatives Reports to: Director of Special Services Funding: Recovs Grant Funded; Independent Contract; $350 Daily Rate for Master’s level INSTRUCTIONS TO APPLICANTS: External Candidates - Submit letter of interest and resume on-line to: https://www.olasjobs.org/PeekskillCitySD If you are a Peekskill resident and/or a Peekskill Alumni and are interested in applying, please submit a letter of interest and resume to Mr. Lewis, Assistant Superintendent for Administrative Services at hr@peekskillschools.org Please do not submit additional materials via: e-mails sent directly to School District employees; the U.S. mail; hand-delivery; or fax machine.
