GENERAL DESCRIPTION:The School Social Worker is responsible for providing all required counseling mandates as found on each student's Individualized Education Plan as well as working with school staff, outside agencies and each child's parent/guardian to ensure the appropriate development of each student while at Martin De Porres School.MAJOR DUTIES AND RESPONSIBILITIES:Provide all required student counseling mandates as indicated on each child's IEPProvide counseling either on an individual or a group basis as prescribed on each student's IEPMaintain all necessary related service counseling attendance cards and log notesas required to meet Medicaid reporting mandatesEnsure that the school's policies and procedures regarding Child Abuse and Neglectare understood and implemented on behalf of all students at Martin De Porres SchoolEnsure that all intake screening requirements are met upon enrollment of a new Student at Martin De Porres SchoolProvide input on monitoring students taking psychotropic medicationProvide feedback to prescribing doctors for students taking psychotropic medicationProvide families with information and direct assistance in seeking needed emergency servicesMaintain contact with out-of-school service providers and link follow-up and advocacy services as deemed appropriate and necessaryProvide instructional staff with ongoing consultation around relevant group and individual classroom management needs, medication, relevant personal, psychological, and family data that affects school functioningProvide administrative staff with consultation around relevant system-wide issues in the school and community/family milieu of the studentsProvide updated social histories as required to better serve a student and increase understanding about the family history, family dynamics and family pressures influencing the child’s family unit Provide annual progress reports and recommendations to the Committee on Special Education in a format determined by the School PrincipalEnsure that counseling notes, attendance recording and log notes are maintained per session/per studentProvide feedback to teaching and administrative staff during student conference sessionsProvide current information on family functioning during student conference sessionsMaintain contact with outside agencies, prescribing doctors and mental health professionalsProvide observational reports and data on identified students as needed to better serve each studentAttend IEP annual review conferences as determined by the School Principal in consultation with Curriculum and IEP CoordinatorEnsure that home visits are made in a timely manner and at a mutually agreeable time and placeProvide administrative support and presence when a student requires emergency assistance through the 911 system in existence (hospital, police, medical support)Generate a Behavioral Intervention Plan and/or a Functional Behavioral Analysisas required to support and facilitate a discussion with the Committee on Special EducationProvide contact with a student's family as often as required to ensure a defined treatment plan is in place for each studentFacilitate contact with a family to encourage better school attendanceProvide face to face contact when a student requires psychiatric hospitalizationEnsure that Transitional Supports are in place when an older adolescent leaves Martin De Porres School Assist with the Intake Screening process for new student enrollees.Provide parent support groups as determined by the School Principal and at a location mutually agreeable to all parties And all other job related duties as assigned by the Martin de Porres School administration SUPERVISES:StudentsQUALIFICATIONS EDUCATION:NYS Certification as a School Social Worker, LCSW, LMSW EXPERIENCE:Minimum of 2 years experience as a school social worker or as a social worker in a non-school setting SKILLS & ABILITIES:Demonstrated ability to work effectively with special needs studentsDemonstrated ability to work effectively with the parents / guardians of special needs studentsDemonstrated ability to work effectively in a special education day school settingDemonstrated ability to effectively advocate for students in your careDemonstrated ability to ensure student emotional health and safety is guaranteedDemonstrated ability to follow administrative directives and school mandates as per student counselingDemonstrated ability to clearly represent Martin De Porres School during the intake processing of new enrolleesDemonstrated ability to communicate with classroom staffWillingness to serve students beyond the confines of a regular school dayWillingness to assist when a students needs emergency assistance through the 911 system (hospital, police, psychiatric intervention)Willingness to visits parents / guardians beyond the geography of the schoolDemonstrated ability to assist parents and guardians in understanding the academic and socio-emotional needs of their children and developing ways for them to influence home activities that will enhance academic and socio-emotional
