REACH Academy is looking for a Transition Specialist. It is a full-time, 10 month position, with the possibility of extra salary for summer school. It comes with an outstanding benefit package including Health Insurance, Dental Insurance, Life Insurance and the stability of the NYS Pension System.The position works the school calendar, on when there is school, off when school is off. The hours are from 8am - 2:45pm Mondays through Fridays (until 3:30 on Thursdays).The role of a transition specialist is to facilitate the transition of students with disabilities from school to the community opportunities in order to assist students in their development of social, vocational, and adaptive skills. Developing internship sites, functioning as a driver and providing supervision in the community (both in groups and 1:1) are essential. This position will also involve instruction on vocational and transition related topics, management of a caseload including working with families and attending CSE meetings as well as providing technical/logistical assistance to facilitate placement of students with disabilities.BA (preferably in Social Work, Mental Health, Human Services or Psychology); Formal or informal training in: NYSED Transition Planning, Positive Behavioral Intervention System (PBIS; preferred), Job Coaching or Job Development; Experience with digital portfolios.
