The Croton Harmon School District is in need of a Transition Specialist for a part-time position (approximately 20 hours per week) for the 2022-2023 school year. This position has a start date of October 17, 2022.This position is responsible for facilitating students with disabilities to transition from school to the community. This position calls for the collaboration with community service providers and stakeholders for the coordination of supports and services necessary for students with disabilities to transition successfully from high school to post-secondary education, training, employment, or long-term support as mandated by special education regulations.
