Western Suffolk BOCES anticipates an opening for a highly motivated and dedicated LCSW to join its multidisciplinary school-based team as an Adolescent Substance Use Disorder Counselor/Recovery High School Coordinator.Candidates MUST have excellent communication and interpersonal skills. This position will require someone who has experience with motivational interviewing, trauma informed care, skills of anger management, relapse prevention and family systems.Experience: Minimum 3 years prior experience working with adolescents and addiction treatment.Position: This is a 12-month, full time position which includes on call responsibilities with communication to the partner agency and Western Suffolk BOCES Administrators.Hours: 9:00 a.m.- 5:00 p.m. Monday through FridayJob Duties:1. Provide crisis intervention services to students throughout the program2. Monitor program wide data collection3. Review student records and engage in collaborative problem-solving with the Recovery HG interdisciplinary screening team4. Facilitate tours and communication with student’s home district5. Assist in gathering additional documents and resources pertaining to student screening an intake as directed by the building administrator6. Ensure students are engaging in and provided their individual and group counseling services7. Provide consultation and feedback to the educational team regarding SUD, Sobriety, Mindfulness, Trauma-informed care, crisis prevention and de-escalation, etc.8. Participate in student daily assessments (check in/check out, morning transitions, student participation throughout the day, bus arrival/dismissal and classroom check in, etc.)9. Support students with educationally based planning needs and assist with student voice/advocacy10. Work with district Guidance Counselors regarding educational performance, course needs and progress11. Coordinate transition planning for post-graduation plans for students12. Facilitate communication between outside treating providers and school staff relative to medical conditions, current treatment plans, etc.13. Sustain communication and contact with families to foster home/school connectedness and understandings14. Maintains clear records associated with communication15. Coordinate and organize parent meetings16. Coordinate and organize school team interdisciplinary meetings17. Chair re-entry meetings with families, districts and students upon arrival to school after a separation from program for any reason18.
