PERSONNEL BULLETINFOR THE 2023-2024 SCHOOL YEAR: Vocational Transition Coordinator (Secondary Level) Job Description/Responsibilities:The Vocational Transition Coordinator will assist students with disabilities and their families in obtaining services necessary to transition from high school to a post-secondary setting, whether for post-secondary education, employment, or long-term support. In doing so, they will engage in the following:Provide direct instruction/support to students with Individual Education Plans (IEP) at the secondary level that are designed to meet their individual transition needs.Develop curriculum, design, and implement lesson plans, and assess student progress.Review and implement transition assessments used with students with IEPS to assist decision making about instructional programming, annual goals, and the development of measurable post-secondary goals.Engage in evidenced-based practices and professional and peer support that will assist participants to empower their students to take a lead role in their education by advocating for themselves and their futures.Disseminates information to staff, students, and parents regarding student needs, progress, and/or resources.Coordinate transition activities for college, career, and/or community readinessManage IEPs for assigned caseload, write IEPs (transition area sections, exit summary), and communicate with general/special education teachers and support staff.Facilitate referral, application, eligibility process for students and families to various agencies (i.e.-DSS, SSI, OPWDD, AccesVR).Have familiarity with the requirements of; Skills and Achievement Commencement Credential (SACC); Career Development and Occupational Studies (CDOS) / Work-Based Learning Standards; Regent’s diploma.Assist with alignment of transition continuum of services at the secondary level. Requirements: Current and valid New York State Certification in one of the following areas:Special Education/Students with DisabilitiesSocial WorkerPsychologistMinimum of 1-2 years of prior experience as a special education teacher, social worker, psychologist within vocational transitional track (coursework and experience).Understanding of disability areas under IDEA.Knowledge of how disabilities impact an individual’s ability to function in the post-secondary setting.Knowledge of state and federal laws/regulations that protect individuals with disabilities in education, employment, and community. Type of PositionProbationary position: tenure area will be based on certification.Start Date of August 30, 2023.Must be willing to attend New Teacher Orientation on August 23 and 24, 2023 (8:00 a.m.-3:00 p.m.).
