WEST BABYLON SCHOOLS ANNOUNCEMENT OF VACANCY 2024-2025 Transition CoordinatorPurpose Statement The Transition/Vocational Coordinator is a leadership role dedicated to ensuring that students with disabilities achieve successful post-school outcomes. This position involves coordinating and facilitating comprehensive transition plans that connect students with vocational and developmental training, as well as financial and residential programs. The Transition/Vocational Coordinator must be a strategic leader with expertise in special education, vocational training, and community partnerships, committed to empowering students to reach their full potential in life beyond school. Reports to: Director of Student Services 611 Grant Stipend: $16,320. Requirements: NYSED Special Education certification and/or administrative experience with career development and post-school outcomes for students with disabilities preferred Duties and Responsibilities: Development of Individualized Transition Plans:Collaborate with students, families, educators, and service providers to develop tailored transition plans that align with each student’s strengths, preferences, and goals.Conduct assessments to identify students' vocational interests, skills, and support needs.Set clear, measurable objectives for post-school outcomes, including employment, further education, and independent living.Refer students for transition evaluations as necessary. Develop appropriate, measurable vocational goals and objectives. Ensure Complete the Transition Services Component of the IEP and contribute to the Summary of Performance. Vocational and Developmental Training Coordination:Organize and facilitate access to vocational training programs, apprenticeships, and internships that are aligned with students’ career goals.Partner with local businesses, vocational schools, and community organizations to create opportunities for hands-on learning and skill development.Provide guidance and support to students in navigating these training programs, ensuring accommodations and modifications are in place as needed.Linking to Financial and Residential Programs:Identify and connect students with financial resources, including scholarships, grants, and government assistance programs, to support their transition to post-school life.Facilitate connections with residential programs and services that offer supported or independent living arrangements based on students’ needs and goals.Refer students to the OPWDD and ACCESS-VR Services as appropriate.Work with families and guardians to understand and access financial planning services for long-term care and independence.Collaboration with Stakeholders:Serve as the primary liaison between students, families, educators, employers, and community agencies to ensure a coordinated approach to transition planning.Host regular meetings and workshops with stakeholders to review progress, discuss challenges, and make adjustments to transition plans as necessary.Educate parents by distributing information on transition issues, hosting parent information meetings, and scheduling tours of adult facilities. Elicit parent participation in program development, implementation and evaluation. Conduct individualized transition planning meetings with parents and/or students. Advocate for the needs and rights of students with disabilities in all interactions with external partners and agencies.Respond promptly to questions and parent concerns.Locate and use community resources to enhance instructional programming.Program Development and Management:Develop and implement programs that support the transition from school to adulthood, including job readiness workshops, life skills training, and career counseling.Monitor and evaluate the effectiveness of transition programs, using data to inform continuous improvements and ensure that programs meet students’ needs.Coordinate supervision and evaluation of students in vocational training sites. Secure in-school and community job placements for students ages 14-21.Manage budgets related to transition programs, ensuring responsible allocation of resources to maximize student outcomes.Formally evaluate staff consistent with NYS requirements. Formally evaluate the effectiveness of the vocational training program, including job coaches, job sites, travel training and prevocational learning opportunities. Participate with the Director of Student Services in the development of the program budget by evaluating the financial needs of the program budget by evaluating the financial needs of the program and making recommendations for adequate funding. Compliance and Reporting:Ensure all transition plans and activities comply with federal, state, and local regulations, including the Individuals with Disabilities Education Act (IDEA).Maintain accurate records of students’ transition activities, progress, and outcomes.Maintain required paperwork including but not limited to Exit forms, Level 1 Assessments, transportation notices, student data, and classroom correspondence.Prepare and present reports on the effectiveness of transition services to school administration and other stakeholders.Submit reports and other required information to the Director of Student Services by the dates assigned.Professional Development and Continuous Improvement:Stay current with best practices in transition planning, vocational training, and services for individuals with disabilities.Provide training and support to educators, staff, and families on transition-related topics.Continuously seek out new opportunities and resources to enhance transition services for students with disabilities.Spend a minimum of 50% of work time in providing instructional leadership to staff assigned to the programProvide effective guidance and technical assistance to staff, especially to those in need of improvement of performance.Keep the Director of Student Services informed of program, staff, facility and equipment needs.
