Position Summary: Anticipated opening pending Project AWARE formal notice of funding.We are seeking compassionate, qualified behavioral health professionals to provide Medicaid-billable services under New York State-approved programs. The position involves direct service provision to Medicaid-eligible individuals with behavioral health, substance use, or chronic medical needs, with a focus on person-centered, trauma-informed care. Key Responsibilities:Provide Medicaid-reimbursable services such as assessments, care coordination, psychoeducation, counseling, skill-building, and supportive interventions in accordance with program guidelines.Conduct comprehensive assessments and develop individualized service plans (ISPs) based on client strengths, needs, goals, and preferences.Deliver services in community settings, homes, schools, or via telehealth as appropriate.Maintain accurate, timely documentation in the Electronic Health Record (EHR) in accordance with NYS Medicaid and agency standards.Collaborate with a multidisciplinary team, including pupil personnel director, primary care providers, behavioral health specialists, and care managers.Participate in supervision, team meetings, and continuing education as required.Ensure compliance with all Medicaid billing regulations and documentation standards.Prevention education in elementary classrooms with evidence validated supportElementary Tier 1 and 2 support for Social-Emotional LearningQualifications:Valid and active NYS license or credential in one of the listed disciplines.Experience working with Medicaid populations, including children, adolescents, adults, or families.Strong knowledge of behavioral health principles, trauma-informed care, and person-centered planning.Familiarity with NYS Medicaid programs (e.g., HCBS, CFTSS, Health Homes, CORE) is preferred.Strong organizational and time management skills.Proficient in using EHR systems and Microsoft Office Suite.Graduate degree in school counseling or social work preferredAdditional Requirements:Valid NYS Driver’s License and access to reliable transportation (if community-based services are provided).Background check and fingerprinting required prior to employment.Being bilingual (English/Spanish or other) is a plus.
