The Middle Country Central School District is seeking a Long Term Substitute Transition Specialist to serve as a connection between the district and outside agencies, including OPWDD, Care Coordination Agencies, ACCES-VR, and the Department of Labor. The liaison will assist families with applying for and coordinating services for individuals with developmental disabilities, oversee district transition programs for students in grades K–12, and plan and implement transition related activities. Anticipated dates from 12/1/25-5/1/26. Candidates need to hold a valid NYSED special education, psychologist, or social worker certification.
