The Mahopac Central School District is seeking a Physical Therapist District wide. The successful candidate will have following responsibilities and qualifications:Key responsibilities of the Physical Therapist include:Performs physical therapy intervention upon the written referral of a licensed physicianParticipates with other personnel in the formulation of student’s IEPRecords all physical therapy evaluation, re-evaluations, progress notes, consultation and discharge summaries on students' records; Maintains program records and statistical reports of the frequency and types of treatment and number of students treated for progress monitoring and Medicaid billingQualifications:Valid New York State Physical Therapist licenseBachelor/Master’s Degree from an accredited programMust have a National Provider Identifier (NPI) number to assist in medicaid claims Salary: As per MTA Bargaining Agreement Location: District WideApplication Deadline: 4/25/2025NOTE: Candidates will have met Civil Service Requirements for this position and be reachable on the current Civil Service list in the title of Physical Therapist.You can view the Putnam County job description found here:https://www.putnamcountyny.gov/component/zoo/item/physical-therapist?Itemid=495
