The LaSalle School is seeking an Community Services Provider/Community Based Case Manager (Full-Time, 37.5/hours per week, non-exempt). This position will report to the Clinical Supervisor and/or Associate Director and Director of Clinical Services. This position will support the Albany, Schenectady and surrounding counties. Work is mainly on the road.Our ideal candidate is energetic; has strong clinical skills of assessment, engagement, and therapy; understanding the community based services and connections to those agencies; excellent time management, computer, interpersonal, organizational, written and verbal skills; collaborative by nature; working with diverse populations of clients; knowledge of best practices, evidence informed practices, and cultural competency; well developed engagement and problem-solving skills; employs Trauma Informed Practices and crisis management skills. Training will be provided by the agency for TCI training.Position Overview:The Community Services Provider is responsible for delivering Community Psychiatric Supports and Treatment (CPST) and Psychosocial Rehabilitation (PSR) services, and completion of related documentation, within the Children and Family Treatment Support Services (CFTSS) array, which includes Intensive Interventions, Crisis Avoidance, Intermediate Term Crisis Management, Rehabilitative Psychoeducation, Strengths-Based Service Planning, Rehabilitative Supports, Personal and Community Competence, Social and Interpersonal Skills, Daily Living Skills, and Community Integration activities.About Children and Family Treatment Support ServicesChildren and Family Treatment Support Services (CFTSS) are new mental health and substance use services, available with NYS Children’s Medicaid, to give children/youth (under age 21) and their families the power to improve their health, well-being and quality of life. These services strengthen families, and help them make informed decisions about their care. Services are provided at home or in the community.Job Responsibilities (only illustrative and not all inclusive):Collaborate with family and service providers to identify goals and objectives, and establish and implement the Treatment Plan, based on the child and caregiver’s needs and preferences.Coordinate with the child and caregiver on scheduling and provision of services.Complete all required documentation within required timeframes.Facilitate service provision and attend relevant meetings as required.Share information related to delivery of services with stakeholders as required.Provide comments and information to collateral contacts for the purpose of Treatment planning, assist in tracking progress.Attend all required trainings.Meet regularly for clinical supervision.Minimum Qualifications (Experience & Education):This position requires that the candidate has a Bachelor’s degree in social work, psychology, or in related human services or children’s mental health, addiction, and/or foster care/child welfare/juvenile justice, plus at least one year of applicable experience;Requires a valid NYS driver’s license.
