Position: Special Education Transition Coordinator 10.5-month work year) *Previous applicants need not apply QUALIFICATIONS: Eligible candidates require administrative certification in school leadership (SDA/ SDL). NYS Certification as a Coordinator of Work-based Learning Programs is preferred. Candidate must demonstrate knowledge of the Career Development and Occupational Studies (CDOS) Commencement Credential. Knowledge of Special Education and transition planning is required. REPORTS TO: Assistant Superintendent of Special Services GENERAL FUNCTIONS:The Special Education Transition Coordinator will work under the supervision of the Assistant Superintendent of Special Services and will be responsible for transition services for students with disabilities from the ages of 14 and up, alternate pathways and graduation options for student with disabilities. The Special Education Transition Coordinator will organize and implement all phases of the Career Development and Occupational Studies (CDOS) Commencement Credential including record keeping of attainment of the Career Development and Occupational Studies Standards, commencement level career plans, work-based learning (WBL), employability profiles, student exit summaries, independent living skills, and community participation for students with disabilities. The transition coordinator will also assist the District Coordinator of Guidance, and postsecondary representatives in defining needs, developing plans, establishing goals, and formulating policies and procedures for program development.SPECIFIC DUTIES AND RESPONSIBILITIES:The Special Education Transition Coordinator will:Support the vision for the program that is articulated by the Assistant Superintendent for Special Services and aligned with the vision for the district. Assist the Assistant Superintendent of Special Services, District Coordinator of Guidance, and postsecondary representatives in defining needs, developing plans, establishing goals, and formulating policies and procedures for program development. Collaborate with all stakeholders on all aspects of the Career Development and Occupational Studies Commencement Credential. Implementation and record keeping of Career Development and Occupational Studies Standards within the high school program. Plan the Coordination of work-based learning experiences for SWD within the community and in the school district. Recruit new businesses and organizations to provide quality work-based and school-based learning opportunities for students. Maintain a list of businesses/industries willing to participate in work-based learning. Prepare and submit required reports and other documents. Assist in screening, interviewing, and selecting qualified students for WBL experiences. Coordinate cooperative education, internships, job shadowing, service learning, and other WBL programs. Prepare commencement level career plans for CDOS Commencement Credential SWD. Develop employability profiles, student exit summaries for CDOS Commencement Credential SWD. Prepare student Exit summaries and chair annual exit meetings/attend AR meeting for classified projected graduates. Provide support to families with transition services such as Adult Career and Continuing Education Services-Vocational Rehabilitation (ACCES-VR). Attend Suffolk County Interagency Transition Team (SCITT) Meetings.
