Early Intervention (EI), birth to 3 year old program. To provide physical therapy services to patients as written by the IFSP, including: Assessment, evaluation and development of therapeutic/rehabilitative/functional goals; To provide instruction in use of equipment; Assessment of the need for devices; Collaborate with other disciplines, as needed in order to address goals and objectives as indicated in each student's IFSP. Sessions will be within home, school, and other community settings. Physical Therapists will instruct family members when appropriate to assist the student in reaching established goals. In compliance with state and local requirements, each therapist will be responsible for maintaining appropriate documentation regarding student services and subsequent needs.Must be NYS licensed/certified and have experience with Early Intervention population. Dept. of Health provider approval is needed to take on EI cases. We invite contractual participation by Certified Minority or Women owned Business Enterprises (Certified M/WBEs)
